So you want to play ?? Read On.....

Download your registration form here in .pdf format

NOTE: You will need to have a program called Acrobat Reader to view the form. If you do not already have Acrobat Reader, you can download it FREE from the Adobe Acrobat website - click here to go to the download site.

Once you have completed your form please hand it to the secretary (or coach) at your next training session.

Please read On.. This explains Millwalls Registration Process

We, as a Club, have sworn to uphold the rules, regulations and any other nonsense (and believe me, there’s a lot of nonsense) that our constituent bodies throw at us. For example, we are allowed to play registered players in league and cup 1 st XV fixtures. To ensure players only play for one club at a time the Constituent Body (in our case, Essex RFU) gives us loads of forms to fill out if you want to get registered.
You must be registered to play for the 1 st XV – it usually takes about 3 weeks from completing the forms if all the details are correct and the passport photocopies (if required) are supplied.

You can play for the Llamas,Third or 4th XV in the meantime. Please get yourself registered, even if you think you will not be playing for the First XV. Registration is FREE and easy to cancel at any time. It is not a ‘contract’ with the Club

The forms (which you can download above) are supposed to be self-explanatory but we have provided guidance notes on the following pages to help you. If you can fill them out by yourself please do so, and don’t forget to sign them. If you are unsure of anything then please email our club secretary Olly secretary@millwallrugby.com), or talk to any Committee member who will point you in the right direction.

IMPORTANT :

In the case of people born outside the UK (even British Passport holders) we need a photocopy of the passport page with your photo on. If you have a VISA of any sort entitling you to be in the UK we also need a copy of that page too. Below are all the questions required on the various Registration Forms.

Your Surname: Your First Name(s) – [yes, middle names required as well]:
Your Date of Birth (dd/mm/yy) :
Your Place of Birth (i..e. name of town/city) :
Your Country of Birth: Your address (in the UK) :
Your Postcode :
Your Phone Number (at least one of the following): Home Phone : Fax Number: (if you have one) Work Phone : Mobile Phone :
Your Playing Position :

You will need to answer the following five questions for each club you have previously played for…
Club Name:
Union that the club belonged to (i.e. SARFU (+ the Province), FFR, NZRFU (+ the Province), ARU etc):
Were you Contracted?: (almost certainly, the answer to this is “No”, but we have to ask!)
Were you Registered? (if you aren’t sure, you can say “don’t know”)
What date did you join this club (mm/yyyy)? (approximately is OK)
What date did you leave this club (mm/yyyy)? (approximately is OK)

Help with RegistrationGetting new players registered with the club is our biggest administrative headache each year. However, once you are registered you do not need to register again each season – you remain a Millwall player until you complete the forms to register for another club.
Please help the Club by getting yourself registered now. It’s free and involves no commitment to play. The forms that you need to fill in, and the information required, are different according to your Nationality and/or Place of Birth.
If you have dual nationality, please complete the forms using your British or EU passport details. This will help the club, as you will not be counted as an “overseas player”.
